Annual W9 Forms are required only for spiffs paid directly to the employee. Click here if you need to securely send a W9!

This form is for secure submission of your spiff incentives so that your personal information is not shared via email. You can enter up to 5 transactions on this form at once. If you have less than 5, simply scroll down and submit at any time. To streamline processing please enter all the requested details.

To enter more than 5, simply visit this form again and enter another batch of submissions. If you have questions about this process, please email spiffs@jettacorp.com. If you are eligible but you don't have access to your policy, please contact your CSR or your supervisor.
